OUT OF NETWORK and SELF-PAY PATIENTS

Self-Pay patients will be provided with secure patient portal access and receipts called “superbills” for payments made to MIND HUB will be provided electronically through this portal. Superbills can be submitted to health insurance companies to negotiate direct reimbursement if desired. We do not accept out of network insurance coverage.

Patient Services:

Diagnostic intake evaluations: 50min: $350, 90min: $400

Medication management only follow up appointments: 15 min: $100, 25 min: $200, 45 min: $250, 60 min: $275

**Medication management + psychotherapy follow-up appointments: 15 min: $50, 25min: $140

Second opinion appointments: 50min: $350, 90min: $400

**PLEASE NOTE THAT WE SPECIALIZE IN PROVIDING MEDICAL CARE WITH BRIEF PSYCHOTHERAPY. For optimal outcomes, clients are encouraged to also work with an individual psychotherapist.

5 - 10 min: $25

11 - 20 min: $50

21 - 30 min: $100

31 - 40 min: $150

41 - 50 min: $175

51 - 60 min: $200

Please Note: telephone calls and e-mail consultations are frequently not covered by insurance.

TO AVOID BEING CHARGED: Please advise MIND HUB of your inability to attend your scheduled appointment at least 48 business hours in advance.

Written documents (per 15 min): $25

Returned check fee: $40

Failure to pay owed balance (begins when 30 days past due): $20 per month

Depositions: $250 per hour + cost of travel (if a MIND HUB provider is required to be deposed / to testify in a court of law).

Case management consultation and care coordination for self-pay MIND HUB patients: 15min: $25, 16-30min: $50, 31-45min: $80, 46-60min: $125
